Prepare for the road with our Department of Transportation (DOT) and Commercial Driver's License (CDL) physical examinations

  • What to Bring:

    1) A Valid State Driver's License

    2) List of Current Medications

    3) Glasses or Contacts if you wear them

    4) Any Required Special Forms completed by managing healthcare provider. To be completed within 45 days before DOT/CDL exam.

    5) Well Hydrated to provide a Urine Sample

  • When to see Provider and Complete a Form:

    1) Taking a prescribed medication that is controlled, causes drowsiness, or relieves pain. The prescribing provider needs to complete this form - Medication Form.

    2) If your vision while wearing glasses is not 20/20.

    3) You have a diagnosis of DIABETES or SEIZURE.

    4) You have SLEEP APNEA. You will need to bring your CPAP compliance report to the exam for review.

  • What to Expect:

    Please read "What to Bring"! It is your responsibility to be prepared for the exam.

    During your DOT and CDL physical exam, conducted by a certified medical examiner, you will undergo the following: Vitals Check: Assessment of blood pressure, heart rate, and other key vital signs. Vision Test: Evaluation to ensure your vision meets regulatory standards. Hearing Test: Screening to confirm your hearing is adequate for safe driving. Overall Body Check: Comprehensive physical examination to assess general health and identify any potential issues. Urine Sample: A sample will be collected to test for medical conditions such as kidney disease or diabetes.

    This thorough examination ensures you meet all necessary health requirements for safe and compliant driving.

  • A copy of your certificate will be sent directly to the Montana Motor Vehicle Division to ensure your compliance and streamline the process for you.

    You may also send a copy to the Montana Motor Vehicle Division and request a copy of the completed examination form if necessary for your personal medical records or for an employer.
